Heart to Heart

According to Kelley Ross Pharmacy Group, one in four patients  hopsitalized for heart failure will return to the hospital within 30 days of being discharged.
Washington Care Services has teamed up with Virginia Mason and Kelley Ross Pharmacy to ensure safe care transitions and a consistant contiuum of care for heart failure patients from Virginia Mason. Our mission is to reduce 30-day readmissions to the hospital and improve the safe use of medications when heart failure patients return to home.
